The Dukes of Hazard. That movie was just plain sad and Jessica Simpson was just annoying in that. I used to watch the television shows and I remember it was nothing like them.
I second (or third or so on) the Scooby Doo movies. Those were really just plain sad.
Bewitched was really bad. The one with Nicole Kidman. They completely butchered the original series.
To Kill A Mockingbird. 90% of what was in the book didn't make it into the movie. There were usually 5 different side-stories going on at any given time during the book, while the movie only showed the main plot. The movie didn't even include everything that was in the main plot.
I completely agree with you. I used to be a fan of Dragon Ball when I was younger, so I expected the film to be good or, at least, decent. Unfortunately, it is really painful to watch.
Get Smart was also pretty bad. I'm a huge fan of the old show and I've seen every episode multiple times, so I can officially say that the movie adaptation was nothing short of awful. Aside from the characters' names and the basic premise of a secret government organization, the movie had nothing in common with the show. The chief was way too calm and not at all harsh, Siegfried was British, and Maxwell Smart was too intelligent. The characters were so far off, it's as if the people who wrote the script and cast the actors had never seen a single episode of the show.

I only have seen the first season on DVD and even I know that it was bad. I think it's safe to say that this is due to Get Smart originally being a '60's, er, sitcom, if you can call it that. It was changed to fit the mindsets and trends of modern-day Americans. Less funny, more *BOOM*
It was mentioned before, but yeah, "Eragon" was the worst.
The Golden Compass was also butchered. Dune was even more horrifying. I love David Lynch, but to this day, he still won't own up to directing that movie.
Dragon Ball Evolution. God I wanted to die.
I've heard bad things. One of my best friends from high school was a huge Dragon Ball fanboy. He HATED that movie.
