I guess that's how her creators intended her; all part of the manga she's in.

Rikka seems to be screaming "Noooooo!" Is she screaming to another person or is it just one of her delusions?

She's just Rikka, she's rather dramatic and eccentric, delusions or not.

I see it now. I've just never seen a two-stringed eyepatch before. Apparently they are more common in Japan than anywhere else.
